Liability and Communication Release

LIABILITY RELEASE

I hereby release, indemnify, and hold harmless United Way of Thurston County (UWTC), its partner agencies, organizers, sponsors, supervisors, and any individuals involved in volunteer activities from any and all liability for injuries or damages that I, or a child for whom I am the legal parent/guardian, may sustain while participating in volunteer activities, including those resulting from negligence.

I also release from liability any person providing transportation to or from a UWTC activity.

I certify that I (and any child listed above) am in good health and able to safely participate in all volunteer activities. I certify that I am 18 years of age or older and legally competent to sign this agreement. I have read and fully understand this release.
 

COMMUNICATION RELEASE

I authorize United Way of Thurston County (UWTC) to photograph, record, and use my image or likeness—and/or that of a child for whom I am the legal parent/guardian—for nonprofit promotional and communication purposes.

I grant UWTC permission to edit, reproduce, distribute, or publicly display these materials without compensation. I waive any right to review or approve the finished product.
 

LIABILITY RELEASE
I hereby release, indemnify, and hold harmless United Way of Thurston County (hereafter referred to as UWTC) the organizers, the agency at which I, or child I am the legal parent or guardian of, volunteer and sponsors and supervisors of all activities, from any and all liability for any injury I, or children I am the legal parent or guardian of, may suffer (including any injury caused by negligence) in conjunction with volunteering on the above noted date. I likewise hold harmless from liability any person transporting me or the children I am the legal parent to guardian of to or from any UWTC activity. I certify that I and the child I am the legal parent or guardian, of are in good health and able to participate in the volunteer activities on the above noted date. I have read the foregoing release, authorization and agreement, and warrant that I fully understand the contents thereof.
COMMUNICATION RELEASE
I hereby waive any claim to the right to the video and/or photographic recording(s) made of me or children i am the legal parent or guardian of by UWTC or its agency(s) to said UWTC. I hereby authorize the editing, duplication, reproduction, copyright, exhibition, broadcast, and/or nonprofit use, and distribution of said recording(s) for purposes deemed suitable by UWTC. I hereby waive any right to approve the finished products. I have the foregoing release, authorization and agreement, and warrant that I fully understand the contents thereof.
